Getting started with care

How do I become a patient?

You can apply in person at the clinic or complete a referral through the Ocean form above. You can also call 807-475-8659 to arrange an appointment and ask about registration.

Who can access care?

The clinic’s service directory lists members of Deer Lake, Fort Severn, Keewaywin, McDowell Lake, North Spirit Lake, and Poplar Hill First Nations as eligible. If you’re unsure whether you qualify, please call—our team can clarify before you travel or apply.

What happens at my first visit?

Expect time to talk about what brought you in, your questions, and possible next steps. Ask the team when booking if there is anything specific to prepare for your appointment.

What should I bring?

Bring your appointment details, a list of medicines you take, and any questions you want to ask. Call ahead to check whether a health card, referral, or other documents are needed.

Are there fees? What language is available?

The service directory lists no fees and English-language service. Call ahead to discuss any communication support you need.

Can I attend in person or virtually?

Services are offered at the Thunder Bay clinic and virtually. Call to ask which option is available for your needs. The clinic is wheelchair accessible, with elevator access to the fifth floor. See hours and directions.

How do I change or cancel an appointment?

Follow any instructions in your appointment confirmation, or call the clinic for assistance.

Need urgent help?

The online referral form is not for emergencies. For a medical emergency, call 911.
